REQUESTING MEDICAL RECORDS

We can provide a copy of your child’s medical record at a cost of $0.50 for the first 50 sheets and $0.25 for additional pages. After requesting the records from our office staff, please allow 7 days for this to happen. 

You can request a copy of your child's immunizations record for no cost.
