Sports physicals are often required by middle and high school-sponsored teams or private teams. Often these visits can be done at your child's annual well visit depending on the date of your child's last check up. It is important to let us know your child will need a sports physical when you make your child's appointment.

The VHSL sports physical form is required for each school year and is good from May 1 of the current year through June 30 of the next year. You can print this form and bring it to your child's visit. The parent/guardian must complete everything except Part III (physical exam) of the form. Please fill out the form entirely prior to the visit. The student and parent/guardian must sign the form as well.

Insurance may cover sports physicals. All Virginia Medicaid Managed Care plans do. Ask about the cost when you are making the appointment.
