Office Financial Policy

We are committed to providing the best care possible for our patients and families. The office financial policy is an agreement between the doctors of the practice, the child's parent or guardian, and the insurance companies.   A clear understanding of the financial policy agreement is important to our professional relationship and allows us to provide the best care for our patients within the limits of what their insurance companies have agreed to cover.


Payments

Payment for services is due at the time services are rendered unless a payment plan has been arranged. Insurance plans vary considerably, and we cannot predict or guarantee what part of our services will or will not be covered. It is the responsibility of the parent or guardian to provide accurate and timely insurance information and up to date phone number and contact information.  Inaccurate or untimely information may result in denial or non coverage by the insurance company and will result in the guarantor being responsible.


Billing

All co-payments are due at the time of service. For policies with high deductibles, we may require a portion of the visit up front if the deductible has not been met. Deductibles and coinsurance are the guarantor’s responsibility. We accept cash, checks, HSA and Flex spending cards, and all major credit and bank debit cards.  A 3% service fee is assessed on all credit card payments. A $40.00 fee will be charged for all returned checks and will result in the account being placed on a “cash/card-only basis”. We will accept payments only by cash or credit card until the balance is cleared. 


Outstanding balances are due within 30 days unless prior arrangements have been made with the billing department. A $10.00 service fee will be added to balances more than 30 days past due. A second $10.00 service fee and a request for payment e-mail/text will be added to balances more than 60 days past due. A third $10.00 service fee will be added to balances that remain outstanding more than 90 days and a final request for payment e-mail/text will be issued. Balances not paid in full within 10 days of the date on the final request letter may be turned over to a collection agency. If the account is forwarded to a collection agency, or is deemed non-collectable due to bankruptcy, we will continue to see the child on an emergency basis for the subsequent 30 days. After 30 days the guarantor must find a new place for medical care. 


Non-emergency appointments such as physicals, well-child checks, ADHD med checks, and similar non-acute matters will not be scheduled until an account balance is paid. We realize that temporary financial hardships may affect timely payment of accounts.  If such problems arise, please contact our billing department promptly for payment arrangements and assistance in the management of any outstanding bills.


Note: The accompanying parent or guardian is responsible for full payment at the time of service. It is the parents’ responsibility to work out the payment of a child's medical care between custodial and noncustodial parents. 


Split Bills

On occasion a child will come in for a check up and also receive care outside standard well care.  Examples include acute illness, management of chronic illness, and medication checks.  As a service to our families we can combine these visits into a well check and a sick visit. These encounters will be split billed, meaning billed as both a well visit and an acute care visit, and if the insurance plan requires a co-pay for an acute visit, this encounter will generate a co-pay for the acute component of the split bill. This service avoids patients having to come in for two separate encounters on two different days. 


Referrals

If an insurance plan requires a referral for a child to see a specialist for procedures or for laboratory tests, please allow 24 hours notice prior to the specialist appointment. Only emergency referrals will be completed on the same day. In general, we will not provide a referral for a problem we have not been consulted about first.  

   

Red Flag Policy

We are required by law to request photo identification if you have not been seen in the last six months. This policy is to protect you from identity theft. If your current address does not match the address on your identification, please bring a utility bill to verify your current address.


Notification of New Insurance

Please bring your child’s insurance card to each visit so that we can verify coverage. If your coverage has changed we will copy the insurance card for our files.


Change of Address and/or Contact Number

Please provide our office with the most up-to-date home address, home, work and mobile phone numbers and email address. It’s critical that we have this information so we can contact you with medical information. Please provide your email contact information so that we may send bills via email.
